This preliminary questionnaire helps us prepare for a conversation about OptiLight. Only your doctor can decide whether treatment is appropriate.
Before you begin
A few health and skin questions
We’ll ask about your skin’s response to sun and then 22 questions from our screening form. If an answer needs a doctor’s review, the online questions will stop and you can still arrange a dry eye consultation.
You must bring your completed questionnaire to your appointment.At the end, print it or save it as a PDF. Bring the printed copy or PDF with you. New Path Vision will not receive your answers online.
