Medical insurance

Medical eye problems may be billed differently from routine vision services. Coverage depends on the reason for the visit, services performed, your specific plan, deductible, copay, authorization requirements, and other benefit rules.

Vision benefits

Routine eye exams, glasses, frames, and contact-lens benefits may be administered through a separate vision plan. A vision benefit is not the same thing as medical insurance.

Dry-eye and specialty care

Some evaluations, testing, procedures, products, or in-office dry-eye treatments may not be covered by every plan. The practice can review expected patient responsibility before elective or non-covered treatment is started.

Network participation and individual benefits are different questions. Coverage can vary between plans offered by the same carrier and may depend on deductibles, copays, authorization rules, exclusions, and benefit limits.
