Do not send completed forms, diagnoses, medication details, eye photographs, or other sensitive medical information through the general website contact form or ordinary email.

Privacy documents
The website privacy policy explains how the public website handles website-related information. New Path Vision’s Notice of Privacy Practices explains how the practice may use and disclose protected health information, describes patient rights, and identifies the Privacy Officer.
The current Notice of Privacy Practices is available online and from the office in paper form upon request.
